2023-24 Lenape Spring Sports Family ID Registration Link (OPENS February 1, 2024)

Registration and Required Paperwork Details

1.  Any student trying out for a Lenape spring sport MUST register on the above Family ID Register Now link PRIOR to trying out.  Family ID registration will open on February 1, 2024.

2.  Section 5 and 6 of the CIPPE form MUST be signed and dated by your physician after June 1, 2023 and uploaded to Family ID when registration opens.   Directions for uploading Sections 5 and 6 of the CIPPE form are found on the Family ID registration page.

If your child played a fall or winter sport, you must upload Section 7 (Parent recertification form)

If you are having difficulty uploading these forms please contact the Family ID customer support number 1-781-205-2800 and they will help you. 

Common Mistakes:  Paperwork is missing signatures (3 signatures needed- 1 student, 1 parent, 1 physician).  Signatures not dated (All 3 signatures need dates).  Parents upload wrong forms or the same form twice.  Please do NOT upload any other forms other than Section 5 & 6 of the Cippe form.  Do not give paperwork to coaches -  All paperwork must be uploaded into Family ID and will be approved by the Athletic Director.
